I Would like to Reinstate My FL Driver License

Hello, My Florida license is suspended and I would like to reinstate. I'm trying to register but I wasn't mandated by a court or the DHSMV to take a 4-hour course to get my license reinstated and I don't have a due date. The FL Driver License Check website shows I'm eligible to elect driving school. What do I put in these sections?

I have a traffic ticket from Dade County Florida. I just completed my court required 4 Hour traffic school course with your website. My traffic school is due into the court by the the 6th of the month.
